I'm at a loss and not 100% sure, hope you Guys can help me.

Hi,

i live in Germany and my English is only medium, so I will do everything through google translator. So forgive me if some sentences are not grammatically correct.

I'm very clueless and not 100% sure if I have PSSD or not, hope you can help me.
When you ask doctors, they always say it's a matter of the mind or you should reduce some stress and then it would work again.

To my story. I am 35 years old and my sexual problems started in 2014.
Before that everything was fine with me, everything worked great.
I often struggled with acne when I was young and had tried everything possible to get rid of it.
In 2011 I went to a dermatologist and he prescribed isotretinoin for me to take as a low-dose therapy. Then i took this isotretinoin until 2016, good 5 years, first 20mg every day, then every two days, then only 10mg every two days, the last year then only once a week. My skin got better and there were no more pimples.

I have to add one more thing.
I also took mirtazapine in 2008 because of my insomnia, but only a small dose to use the tiring side effect.
I only took mirtazapine daily for 1 month and only a very small dose, 7.5 to 3.25 mg a day.
After that I only took mirtazapine as needed, there were even months in between when I got along without Mirta.

Anyway. Until recently, I didn't even know that mirtazapine and isotretinoin are suspected of being able to trigger PSSD.
As mentioned, my sexual problem only started in 2014, so before that everything worked perfectly with mirtazapine and isotretinoin.

I will now briefly describe my problem, or how exactly my sexual problem manifests itself and whether this is an indication of PSSD.

Somehow my libido has totally flattened out, more precisely the sexual desire.
Otherwise everything seems to be working. I can feel my penis and enjoy the stimulation, also the orgasm, but somehow there is no horniness.
Optical stimuli attract me but only very flattened. Sometimes I can get an erection just like that, but sometimes I have to stimulate my penis myself to keep it stiff.
So my problem is rather simply the lack of horniness. The horniness so that you can fully enjoy the sex, not just the stimulation and the orgasm at the end.
I'm horny, if at all, once a month if I'm lucky.
Is it something mental for me or do you think it could be PSSD?

I was seeing an endocrinologist and when I told him that, he just looked at a loss.
I still have an appointment with a urologist but I know he won't be able to help me either or he'll just prescribe Viagra like all urologists do.
A blood count was also made at the family doctor and the values are all fine.

I then registered here, because who knows better about this disease than those affected themselves.

As I said, I'm not quite sure, but since I've now read that isotretinoin and mirtazapine can trigger PSSD, it has made me very insecure and could explain why I've had this problem since 2014.

Even when I was at my most stressful or when I wasn't doing well mentally, I was always horny when I saw women I liked.

So how can it be that this horniness is suddenly gone and only very rarely occurs.

Maybe you can enlighten me, I would be very grateful.

Yes blood tests were done.
The blood test revealed that my thyroid level is slightly elevated, but that wouldn't cause any sexual problems, according to endocrinologists.

Otherwise my blood levels are fine.
I don't know if the urologist will do another blood test. But the deadline is next year.

Thyroid can be really important in sexual life especially if you are on hypo side. Have you done an ultrasound? Can you post your thyroid blood test levels?

As already described by the endocrinologist, the values are not really high.

Yes, they did an ultrasound and everything is fine.
The values show a slight underfunction.
The values should be between 0.27 - 4.20.
My score is 5.06.

My friend i am not an endo doctor but really find a good one...

You are subclinical hypothyroid... Your optimal tsh values must be from 0.5 to 2. Many people with tsh values over 4 have symptoms ... Do you feel tired? Or you gain weight easy?
